1. Medical Disclaimer

• Services provided at MedLounge San Ramon are elective wellness and aesthetic treatments and are not a substitute for medical care from your primary physician.

• Results vary from person to person, and no guarantees can be made regarding outcomes.

• All procedures carry risks. Your provider will review these with you during your consultation, and you must sign informed consent forms before receiving treatment.

• Always consult your healthcare provider before beginning any new wellness or aesthetic treatment, especially if you have underlying medical conditions, are pregnant, or are breastfeeding.

2. Eligibility for Services

• Clients must be at least 18 years of age to receive most treatments, unless otherwise permitted with parental/guardian consent.

• We reserve the right to decline services at our discretion if we determine treatment is not medically appropriate or safe.

3. Appointments & Cancellations

• Appointments must be scheduled in advance and are subject to availability.

• A credit card on file is required to hold your appointment.

• We kindly request at least 24 hours’ notice for cancellations or rescheduling.

• Cancellations with less than 24 hours’ notice or missed appointments may incur a cancellation fee of [$25].

4. Payments & Refunds

• Payment is due at the time of service.

• By receiving treatment, you acknowledge and accept the risks, possible side effects, and the need for multiple sessions to achieve optimal results.

• MedLounge San Ramon and its providers are not liable for complications arising from undisclosed medical history, medications, or failure to follow aftercare instructions.
